BREAST CANCER AND NEOPLASTIC MENINGITIS: DEATH REGISTERED AT A REFERRAL HOSPITAL IN GOIANIA-GO, BRAZIL, FROM 2004 TO 2013

Institution: Centro Universitário de Anápolis – UniEvangélicaInstitution: Centro Universitário de Anápolis – UniEvangélicaOBJECTIVE: The main goal is to characterize the death cases caused by breast cancer (BC) and neoplastic meningitis (NM) at a reference hospital for oncological care in Goiania-GO, Brazil. METHODOLOGY: Retrospective analysis of medical records with death registry of patients with BC and NM at a period from 2004-2013. RESULTS: From a total death records of 366 caused by BC, 14 (3.8%) had a diagnosis of NM, all female. The majority of NM diagnosis (85.7%) occurred from 2012. The average age at NM diagnosis was 49.2 years. According to anatomopathological examination, 85.7% had invasive ductal carcinoma, of which 66.7% as grade 2. The hormone receptors were positive in 92.8% of patients, of which 28.6% were positive to Human Epidermal growth factor Receptor 2 (HER2). Almost 43% presented stage IV at the period of BC diagnosis. The majority number of cases metastasize before diagnosis of NM, of which 64.3% of patients had bone metastasis, 28.6% had liver metastasis and 21.4% had lung metastasis. In four patients the location of the first metastasis was diagnosed in the meninges. In the NM treatment, 57.1% of patients received total brain radiotherapy, 41.7% systemic chemotherapy and 1 patient was treated with intrathecal chemotherapy. The estimated survival rate from the diagnosis of BC was 3.97 years and the survival rate after the NM diagnosis was 7.3 months. The average time between diagnosis of BC and the diagnosis of NM was 3.42 years. CONCLUSION: The diagnosis and treatment of NM in patients with BC need more clinical attention and further study in the state of Goias This condition was probably underdiagnosed in the evaluated years. The intrathecal chemotherapy treatment, despite being one of the most common treatments, was shown in only 1 patient. Keywords: Meningeal carcinomatosis, breast cancer, oncological care.
