Serviço de Mastologia, Hospital Materno Infantil – Goiânia, GOPurpose: case report of high grade fusocellular mesenchymal neoplasia. Case report: A 48-year-old female patient comes to the Mastology Service of Hospital Materno Infantil, in follow-up since 2013, referring to progressive growth of the left breast nodule for 3 months, progressing with ulceration and purulent discharge. A history of ipsilateral fibroadenoma nodulectomy in 2003 and 2015. On examination, mean breasts, bleeding ulcerated periareolar lesion of about 6 cm in diameter in the upper outer quadrant (UOQ) of the left breast, associated with fibroelastic consistency of about 10 cm in diameter, in addition to lymph node palpable in the ipsilateral armpit (N1). The core-biopsy showed low-grade fusocellular mesenchymal neoplasia, olagenic stroma with areas tending to hyalinization, and a focal area of giant foreign body type reaction. Patient underwent mastectomy and immediate reconstruction with TRAM, without intercurrences. The anatomopathological of the surgical specimen, associate with the immunohistochemical, demonstrated a high degree of fusocellular malignant mesenchymal neoplasm, measuring 8.5×7.0x7.0cm, with the following markers: keratin cocktail (AE1 / AE3) and vimentin diffusely positive; Ki 67: 15%; cytokeratin 8/18 positive in rare isolated cells; negative desmin, CD-34, estrogen receptor and HER2. Patient presented good postoperative evolution, being in adjuvant treatment. Conclusions: Primary sarcoma of the breast is a rare tumor, the predominant clinical aspect of which is a rapidly growing painless and mobile mass, with an average size of 5 cm at diagnosis. For the diagnosis with frequency, it is necessary to associate the histopathological findings with the immunohistochemical study.   breast cancer, Reconstrução mamária
