Serviço de Mastologia, Hospital Materno Infantil – Goiânia, GOPurpose: report a case of benign phylloides tumor of the breast in an elderly patient. Case report: A 75-year-old woman, referred to the Mastology Service of Hospital Materno Infantil, referring left breast nodule growth in the last 3 months. Background of left fibroadenoma nodulectomy in 2000 in another service. Clinical examination: a large tumor occupying the entire upper outer quadrant (UOQ) of the left breast, with hyperemia of the skin; armpits without changes. Mammography: hyperdense nodule, regular and lobulated contours, located in the left breast UOQ. Ultrasonography revealed a massive hypoechoic mass, regular and lobulated contours, showing cystic areas and posterior acoustic reinforcement. Patient submitted to core-biopsy with histopathological examination showing mesenchymal lesion with myxoid pattern, minimal nuclear pleomorphism and no identifiable mitosis. Patient underwent mastectomy. Histopathological examination, associated with the immunohistochemical findings (AE1 / AE3 vimentin positive, desmin, CD56, CD34 negative, Ki67: 10%) showed benign phylloides tumor with extensive myxoid stromal area without evidence of atypia. Patient evolved well in the postoperative period, with no signs of recurrence 7 months after surgery. Conclusions: Phylloides tumors of the breast are uncommon fibroepithelial neoplasys. They may occur at any age but are more common in the 35-55 age group (approximately 20 years later than fibroadenoma). The most common clinical presentation is a palpable nodule, non-painful, mobile, well-defined and rapidly growing limits. The size can vary from 1 and 50 cm. Clinical and imaging diagnosis is difficult. The presented case draws attention to the age of the patient. breast cancer, TUMOR FILODES
