“Objective: To compare the aesthetic result and the security profiles of breast surgical incisions treated with 2-octyl cyanoacrylate versus intradermal suture with nylon. Methods: We randomly assigned women with breast lesions to undergo a tissue adhesive or suture with nylon 4.0 after the breast surgery. Also analysed the size of surgical wounds and breast lesions, the operative time of skin closure and the preoperative values of haemoglobin and leukocytes, as well as the age, weight and height of patients. The cosmetic outcome was assessed after 40 and 180 days after surgery. Results: There were 37 patients randomized to OCA group and 42 to suture group with nylon thread. The study was stopped before the end of the recruitment of patients for the occurrence of more dehiscence in OCA group (OR 11.42, 95% CI 1.36 to 96.02, p = 0.007). There were no significant differences in surgical time and the aesthetic result postoperatively between the two groups. The average size of the wound was higher in OCA group compared to the suture group [32.97 (+10.54) mm vs 27.64 (+9.56) mm]. However, there was no correlation between the size of the wound and the largest number of dehiscence (p = ns). Conclusion: Octyl-2-cyanoacrylate showed higher risk of dehiscence compared to intradermal suturing with equivalent aesthetic results.
